When you use that definition, the difference of quotients, you have to take the change in position with respect to 2 points. The second point is the point f(x + h), that is some distance "h" from the original point "x". The smaller you make h, the better approximation of the tangent you will have. That's why when you use that definition, they put it in a limit where h ---> 0 (that is, the smaller h becomes, the smaller the secant line, the smaller the difference between a secant and tangent!).
